Volunteer Release Forms

    Thanks very much for taking the time to complete our volunteer application process! For applicants under age 18, the following sections must be completed by a parent/guardian. Please contact us if you have any questions. 
    Liability Acknowledgement and Release 
    Acknowledgement: I hereby understand and acknowledge that there will be activities that incorporate discipline and supervision during participation. I additionally understand and acknowledge that the instructors are mature and intelligent and will use wisdom and caution to minimize the possibility of accidental injury. However, because of the type of activity involved, I also understand and acknowledge that the prospect of bodily injury while participating in the activities at NORTH LITTLETON PROMISE is a possibility even under the most stringent and safe conditions.

    Waiver: Having understood and acknowledged the above, I hereby waive any and all of my rights pertaining to any and all liability for injuries that are a proximate result of participation in the said activities that are not against public policy, in relation to NORTH LITTLETON PROMISE.
    Authorization to Use Photographs and/or Audio-Visual
    I, the undersigned, hereby authorize NORTH LITTLETON PROMISE to use, reproduce, and/or publish photographs and/or video that may pertain to me—including my image, likeness and/or voice and/or written testimony without compensation. I understand that this material may be used in various publications, public affairs releases, recruitment materials, broadcast public service advertising (PSAs) or for other related endeavors. This material may also appear on the Organization's or project sponsor's Internet Web Page. This authorization is continuous and may only be withdrawn by my specific rescission of this authorization. Consequently, NORTH LITTLETON PROMISE may publish materials, use my name, photograph, and/or make reference to me in any manner that NORTH LITTLETON PROMISE deems appropriate in order to promote/publicize service opportunities.
    Background Check Authorization
    I, the undersigned, give my authorization to NORTH LITTLETON PROMISE or its representatives to verify the information provided during my application process. I verify that the information on my Volunteer Application is true.

    To protect your identity, a staff member from North Littleton Promise will request your birthdate and social security number by phone.

    ​For applicants under the age of 18, references will be called in lieu of a background report.
